Student name: ____________________________
What you have to do
There are a total of four (4) parts to complete, as follows:
Part A – consists of
Multiple Choice
and True/False questions requiring a correct answer for each.
Part B – consists of tasks requiring an extended response answer.
Part C – consists of a task requiring an extended response answer.
Part D – consists of questions requiring extended response answers.
Securely attach your answers on separate sheets that clearly identify the question to which you are responding and number the pages in sequence. At the end of each answer, allow adequate space (at least 6-8 lines), for the teacher’s comments and feedback.
You will be assessed on how well you interpret each task request and how you structure each according to layout, sequencing and by providing all relevant and applicable details for each task.
This is an ungraded unit. Your result is based on the evidence you provide to meet the criteria for competence as specified in the unit of competency and grading criteria. You will receive a result of Achieved Competency (AC) or Not yet Competent (NC).
Information on how to submit your assignment is located on the OTEN website under your OLS log-in.

Task/Question
Part B: Hazard Identification and Risk Assessment
Read the following scenario. You will be required to identify the obvious hazard and conduct a risk assessment using the four-step hierarchical process model.
Scenario
Harry is a nineteen year old casual employee who has been newly appointed at the fast food outlet you work in. He has been there two weeks. Whilst cleaning behind a grill Harry touched an exposed live electrical wire and received a high voltage electrical shock. He was rushed to hospital with severe muscle, nerve and blood vessel damage to his arm and body.
Investigations by WorkCover sparked an immediate inspection of all electrical equipment owned by the fast food chain. The chain pleaded guilty to failure to provide a safe workplace for employees.
1.
Identify the most obvious hazard presented in this scenario.
2.
Conduct a risk assessment for this hazard by using the four-step hierarchical process model. Be sure to include the appropriate matrix (or table) for each step.
Task/Question
Part C: Risk Control
Based on the previous scenario in Part B and the Hazard Identification and Risk Assessment task you completed, you are now required to identify ways to eliminate the risk or take action to control the risk using the five-step hierarchical approach.
Task/Question
Part D: Review and Documentation
Following on from the electrocution incident at the fast food outlet mentioned in
Parts B & C, the outlet has now implemented risk control strategies.
