Click on “Archive” and used the advance search tool to narrow down the breaches to those that occurred in the state of Indiana, from the time period of 1/1/2021 – 12/31/2021, with the covered entity type of “Healthcare Provider”. Review a handful of the breaches before selecting one that is of interest to you. In your initial response, discuss the detail of the breach you selected. Then, discuss privacy and security policy measures that could be implemented to help the provider/organization mitigate future risk to similar breaches.
