Identify at least three (3) types of health care workers/professionals (job titles) that would work within each provider (See second column of Health Care Workforce Matrix in Appendix B).  Information should be thorough and accurate.

Provide at least a two (2) sentence job description for each job title listed in column 2 (See third column of Health Care Workforce Matrix in Appendix B).  Information should be thorough and accurate.
