Every year, millions of people are caught unprepared for automobile-related accidents and

AGING WITH GRACE, LLC

Aging with Grace, LLC (AWG) is a national eldercare assistance program that provides THE solution

to eldercare stress by educating, coordinating and facilitating senior care options one member at a

time. Members have access to a Careington dedicated area on the Aging with Grace website that

includes resource information and a Caregiver Knowledge Base, Caregiver Toolkit, Helpful Tips and

Tools for Managing Caregiver Stress, other resources and links to preferred providers and

information for those who prefer to research on their own.

For members who are or will be providing eldercare for themselves, spouse or sibling, finding and

coordinating elder-care services is no easy task. It requires an in-depth knowledge of complicated

systems such as healthcare, insurance and housing options. In addition, elder caregivers are dealing

with caregiver stress and in many cases financial hardship. With greater numbers of boomers

expected to be caring for elderly relatives in the future, it’s critical to find ways to help members

cope with those demands.

Aging with Grace also offers assistance to Veterans to help determine if they qualify for the Non-

Service Connected Disability Pension Benefit which will help defray the costs of non-reimbursable

expenses such as homecare, assisted living and nursing homes. This service will allow for veterans or

Resources for Living (RFL) provides telephonic counseling and referral services to members to help

balance their busy lives. A RFL counselor has a discussion with the member regarding the best

services for their needs. The counselor does research and then provides the member with helpful,

relevant information about services and programs in their area within three days. Members have

access to real, workable solutions and answers to concerns about daily issues with just the click of a

button or phone call.

RFL can find solutions for child care, elder care and many other everyday personal, household and

family issues, including (but not limited to):

 Child care – Childcare centers, family daycare homes, summer camps, nanny/au pair

agencies, nursery schools, before and after-school programs, kindergarten classes, care for

children with special needs, pregnancy resources and adoption information

 Elder care and care for adults with disabilities – Home health care, rehabilitation centers,

Resources for Living (RFL) provides telephonic elder care counseling and referral services to help

balance members’ busy lives. A RFL counselor listens to situations and discusses services that are

appropriate. The counselor researches the services available to the elder and the geographic area.

The research is completed in two to three business days. Members have access to experienced

elder care counselors and will receive referrals to services that assist members with their care.

Carotid Artery: Stroke is the fourth leading cause of death and the number one cause of disability in the U.S.

Over 80% of strokes are preventable if diagnosed early, even though four out of five strokes do not have

symptoms. LLS uses Doppler color-flow ultrasound to create images of the left and right carotid arteries and

measures the blood flow within them. The screening is searching for plaque that may block the blood flow to

the brain. Clots and plaque build-up cause 87% of strokes.

Abdominal Aortic Aneurysm: AAA occurs when the large blood vessel that supplies blood to the lower part

of the body enlarges or balloons which may cause a rupture. This is the tenth leading cause of death for men

over age 55 with a 90% mortality rate. 30% to 50% of AAAs result in death before the victim reaches the

hospital. Ultrasound technology is used to measure the size of the abdominal aorta to detect the

development of this condition early.

Atrial Fibrillation: AF is the most common type of heart arrhythmia. The heart’s upper chambers beat

irregularly, and blood is not pushed completely out of the heart which can cause a stroke. 15% to 20%

percent of all strokes are caused by AF. An ECG or EKG (electrocardiogram) using electrodes placed on the

wrists and ankles is used to screen for the presence of AF.

Peripheral Arterial Disease: PAD, also known as “hardening of the arteries,” affects about 8 million

Americans. PAD occurs when the large and medium-sized arteries supplying blood to the legs become

narrow or clogged, constricting the flow of blood. People with PAD have a two to six-fold increased risk of

death from heart attack and stroke. Screening consists of measuring the systolic blood pressures in the arms

and ankles to calculate the Ankle Brachial Index (ABI) for each side.

Osteoporosis: This is a serious condition when the bones become weak, increasing the risk of falling and

fractures. In the U.S., ten million individuals have osteoporosis and nearly 34 million are at risk of developing

the condition. Osteoporosis screening consists of a quantitative ultrasound measurement of the heel bone to

estimate bone mineral density.

Health Risk Assessments: Biometrics (blood pressure, lipid panel, fasting glucose, BMI and waist

circumference) and medical history questions are run through a proprietary algorithm to calculate risk level for

six major chronic diseases: heart disease, heart failure, stroke, diabetes type 2, COPD and lung cancer. A

days a week and 365 days a year. NSD has the most extensive and diverse network of road and

tow professionals with access to over 45,000 providers available. The confidence you feel in your

vehicle is elevated by your membership in the NSD Roadside Assistance Plan.

Services Include:

 Towing: Receive a tow to the nearest repair facility (Up to 15 miles). You are guaranteed no

out-of-pocket costs for a tow within 15 miles of your disablement.

 Emergency Fluid Delivery: Water, oil or gas will be delivered to you in the event of an

during any seven-day period will be accepted. Reimbursement for accident towing and services secured through any other source. 6.

NSD will send a letter to any customer who exceeds five dispatched road service calls within the customers yearly membership term

notifying the customer that they will be placed on a “Cash Call Basis,” meaning NSD will still dispatch a service provider to a customer

on a “Cash Call Basis,” but the customer will have to pay the service provider a discounted fee for the service at the time the service is

home country. This product can be packaged with other services or embedded into a tour

package to assure travelers have access to medical services while traveling anywhere in the US.

The product provides up to $25,000 per person for medical care incurred while traveling. Coupled

About Optimal Health Coaching

Telephonic Health Coaches are health professionals with various backgrounds and degrees,

including registered nurses, certified fitness trainers, registered dieticians, smoking cessation

facilitators and certified health education specialists.

Once member completes the online health risk assessment, a personal health coach will help

review and create a health action plan to identify member’s strengths to achieve optimal health.

The frequency and length of sessions are based on member’s risk level and readiness to improve.
