Part 1:
Collaborate on Quality: Issue Analysis and Lea…
BHA4004
Introduction
In this third assignment in the course, you will assume the role of a newly promoted quality
manager at your local hospital. This role requires you to address deficiencies by improving
organizational culture, providing leadership oversight, and cultivating staff relationships
within the organization. While you have many priorities in this new role, one of your first is
to analyze a recent incident that occurred within the organization and to create a
leadership action plan with recommended strategies and tactics to address not just the
specific incident, but to drive safety and quality improvement throughout the organization.
This assignment differs from the first two assignments in that with this, as the quality
manager, your focus is broader. Rather than focusing only on identifying specific actions
the organization can take to remedy a particular incident that occurred, you are
concentrating on what steps you will take as the quality manager to influence the
organization’s leadership to cultivate a fair and just culture. You will determine what
departments, what leaders, and what personnel you will collaborate with to improve
quality for the whole organization. In this type of culture, safety is at the forefront of
everyone’s job and all associates welcome the opportunity to highlight issues—without
fear of reprisal—so that they can be addressed at a systemic level throughout the
organization.

Select one of the three incidents from the Vila Health: Patient Safety simulation.
These are common incidents you are likely to encounter in the health care field.
These included a patient identification error, a medication error, and a
HIPAA/privacy violation. You may select one of the incidents you worked with in
the previous assignments or select another one. Choose one that holds the most
interest for you.
Consider the following analysis questions once you have selected the incident on
which you will focus:
What information do you possess about the issue? (Note: You may
not be able to answer all of these questions; just include the
information you know.) Consider:
▪ Who was involved?
▪ During what process (clinical, communication, or
operational) did the issue occur?
▪ When did the issue occur? During a particular shift? On a
particular day? During peak hours? Under certain clinical
circumstances?
▪ Where did the issue occur?
o What additional data about the incident would you like to collect and
analyze?
o Which best practices were likely avoided that could have contributed
to the issue? (Note: This information will prove useful to you as you
complete your analysis and leadership action plan.)
Consult the readings provided in Week 6.

Instructions
Write an analysis and leadership action plan for the issue you selected that will enable you
to address the issue on an organization-wide basis. Please make sure to include all of the
following headings and answer all of the questions underneath each heading.
•
•
•
Introduction: Issue Summary
o How would you summarize the key elements of the incident that
occurred?
o What is your goal in addressing the issue?
o Which 2–3 key items will be your focus? For example, you may elect
to focus on nursing staffing levels if being short-staffed in nursing is
contributing to compromises to patient safety.
Culture: Explain what culture is and why it is a critical priority for safety and
quality, providing at least two evidence-based strategies for cultivating a culture
of safety.
o What is culture?
o Why is culture a critical organizational priority for safety and quality?
o What do you know about the existing organizational culture, based on
the knowledge you have about the selected issue?
o What are some of the evidence-based strategies you are considering
that could be employed to cultivate a culture of safety?
IHI Triple Aim: Apply the IHI Triple Aim to develop a health care leadership
strategy that focuses on optimizing health care system performance.
What is the IHI Triple AIM?
How does the IHI Triple Aim apply to this specific incident?
What IHI Triple Aim elements will you incorporate into your
organizational improvement strategy?
Leadership and Collaboration: Propose evidence-based leadership and
collaboration strategies to enlist the aid of key organizational leaders in
establishing a safety and quality culture.
o Which key departments need to be directly involved with the
corrective action process?
o What is your rationale for selecting these departments? For example,
you may want to involve nursing because many of errors involve
nurses and obtaining their buy-in is critical to achieving the
organizational priority.
o Which specific senior leader, front-line staff member, and clinical
expert will you include in your action plan and hold accountable for
implementation?
o What are the implications of not engaging with all departments
toward making safety and quality top of mind?
o How might you involve other departments in addressing the specific
issue and the cultural issue?
o Which specific leaders within the organization could assist you in
addressing this issue and in making patient safety and quality top of
mind throughout the organization? Examples for you to consider
include the chief nursing officer, the chief medical officer, the patient
safety officer, et cetera.
o What role do you expect these leaders to play in addressing the
specific issue and the issue of culture?
o What best practices would you employ to enlist their aid in the
improvement effort?
Leadership Action Plan: Create an evidence-based action plan that includes
leadership strategies to establish a safety and quality culture.
o What are three evidence-based (supported by current literature)
leadership strategies you recommend that would help to solve the
incident that occurred?
o What are three evidence-based best practices you recommend to
address the issue on an organizational level?
Opportunities to Enlist Governing Board: Determine opportunities to enlist the
governing board’s aid in fostering a fair and just culture.
o What role does the organization’s governing board have in terms of
quality and safety in the organization?
o How could you enlist the governing board’s aid in your improvement
initiative?
o What additional information could you provide them to increase their
involvement in the organization’s safety and quality improvement
efforts?

Part 2
BHA4006
Compliance Training
At this point in your academic career, you know that health care is a heavily regulated
industry. While applicable regulations may vary from one organization type to the next, the
typical health care organization must navigate through a wide array of laws, quality
standards, regulations, and ethical considerations as part of day-to-day operations. What
this means is that the staff working in these organizations must also fully understand their
organization’s regulatory environment and their role in regulatory compliance.
Successful compliance depends on how well the staff is trained. Managers need to be able
to teach their staff compliance best practices. The compliance content in the training is one
aspect of the health care manager’s job. The other critical aspect of the job is to be able to
convey the compliance content effectively and efficiently. In today’s environment, it is not
enough to simply place a check mark next to the staff trained item on the manager’s to-do
list. Staff members need to be able to retain the compliance best practices they have
learned and apply them on the job. In successful health care organizations, compliance
training does not just need to be done; it needs to be done effectively and efficiently. This is
no easy task, given the complexity of health care regulations.
In this assignment, you will continue your work on a team-based initiative for Vila Health’s
chief compliance officer. For the purpose of this assignment, you will assume the role of
quality assurance officer in your health care organization. You have been tasked with the
responsibility of developing a presentation that prepares the organization’s department
heads to deliver the compliance training to their staff members.
Preparation
Conduct independent research on these topics:
•
•
Regulations, accreditation requirements, and ethical standards that govern the
health care industry.
Current best practices in compliance training and health care industry-accepted
training programs.
Consult the resources in the weekly readings as a starting point for your research. You may
also want to consult the Health Care Administration Undergraduate Library Research
Guide Links to an external site.for additional guidance on how to conduct scholarly,
professional research.
Instructions
Assume the role of a quality assurance officer at one of Vila Health’s community-based
hospitals. You recently received approval from your organization’s senior leaders to
provide compliance training to all staff members based on the new approach to compliance
your organization has adopted.
Because of the number of staff requiring training, you will not be able to deliver the
compliance training yourself. Instead, your organization’s department heads will conduct
the training for their departments. Your job is to develop a presentation to help the
department heads train their staff on the organization’s new approach to compliance.
You have scheduled a meeting with the department heads to introduce the compliance
training. For most presentations like this, you will get only 3–5 minutes to make your
points, so you must keep it concise. As you are doing your planning for your presentation to
department heads, you have decided that your presentation needs to consist of two parts:
•
•
Part One: Background on Regulation, Accreditation, and Ethics in Health Care.
o This addresses the compliance content department heads need to
cover in their staff training sessions.
Part Two: Compliance Training Effectiveness.
o This addresses how to design and deliver compliance training that
staff members retain and apply on the job.
You have determined you will need 10–12 slides to cover the content you plan to deliver to
the department heads. You have also created an outline of what specifically you need to
include in your presentation. Here is the outline you created:
Part One: Background on Regulation, Accreditation, and
Ethics in Health Care
In seven to eight slides:
•
•
•
•
•
Analyze regulatory, ethical, and accreditation compliance in health care
(one slide).
o Distinguish accreditation from regulations and ethical considerations
in health care.
Highlight the risks and consequences of noncompliance and ethical breaches (1–
2 slides).
Analyze important health care regulations, such as privacy laws, quality
standards, and patient rights (three slides).
Present key ethical considerations all staff must understand (one slide).
Explain how the concept of continuous readiness applies to compliance
(one slide).
Part Two: Compliance Training Effectiveness
In three to four slides:
•
•
•
Explain best practices for training health care staff on compliance (1–2 slides).
Conclude with a final meaningful compliance message, such as a motivational
statement or a quote about compliance (one slide).
Include your reference slide with APA-formatted references (one slide).
See Evidence and APALinks to an external site..
Now that you have your outline in hand, develop your presentation to department heads on
the organization’s new compliance training program that they will be delivering to their
staff members.
Additional Requirements
Your assignment should meet the following requirements:
•
•
•
•
Written communication: Your slides need to include bullet points highlighting
the key concepts. Avoid using block paragraphs or pictures only. Make sure your
content is clear, persuasive, well organized, and free of errors in grammar,
punctuation, and spelling.
Title slide: Develop a descriptive title for your presentation of 5–15 words. It
should stir interest yet maintain professional decorum.
Length: 15–20 slides. Slides should be professional and appropriately balance
text and visuals. Avoid making your slides too text heavy. Instead make use of
speaker notes to include additional content.
References and reference slide: Include a minimum of three citations of peerreviewed sources in current APA format on a final reference slide.
Review the Compliance Training Scoring Guide so that you understand how your faculty
member is going to evaluate your work.
Submit your assignment as a PowerPoint presentation. Review the resource, Guidelines for
Effective PowerPoint Presentations [PPTX]Links to an external site. for PowerPoint tips.

Quantitative Research Questions and Methods
Introduction
The quantitative method is the original scientific method. Quantitative research methods
can be subdivided by various designs that researchers use to answer their research
questions. In fact, the wording of the research question must match the methodology and
may contain words that indicate the design. Quantitative research identifies
what happened, but not why it happened. It identifies causation or relationships.
The relationships in a quantitative research study are similar to those used for a qualitative
design. There is still a relationship between the theory, problem, purpose, and the research
questions, hypotheses, and design. This week you will continue to expand your
understanding of this framework by analyzing and evaluating research questions and
hypotheses in quantitative studies. You will also learn about various quantitative research
designs that researchers use to answer their research questions.
In addition, you will continue to analyze the interrelated elements of a research study,
making the connection among theory, problem, and purpose, and research questions,
hypotheses, and design.
Summary
For this assignment, you will craft a quantitative research question based on the HAC you
selected for the Research Problem and Purpose Statements assignment, as well as examine
quantitative methodologies and data collection strategies that would enable you to answer
your research question.
There is no length requirement for this assignment. Most submissions that fully address all
scoring guide criteria will be 2–5 pages long.
Instructions
For this assignment build on the work that you did in the Research Problem and Purpose
Statements assignment.
The assignment has two distinct parts.
Part 1: Quantitative Research Question
For this first part of the assignment, you will formulate a research question that is
appropriate for a research project that will use quantitative methodologies. Remember the
HAC that you used in your Research Problem and Purpose Statements
assignment. Additionally, your research question should align with the problem and
purpose statements that you developed at that time.
Keep in mind that a well-written research question should be a single statement.
When examining or creating qualitative research questions, the following checklist may be
useful.
Quantitative Research Question Checklist
•
•
•
•
•
•
•
•
Does the purpose statement fit logically with the problem statement? Does the
purpose statement use similar words to the problem statement, or does it seem
to address a different topic?
Based on the problem statement, do the research questions align with the
method and design of the study? For instance, are words like perception used
that would automatically reject a quantitative study? Reference the research
methodology publication to ensure alignment.
Do questions seek to describe responses to the variables described in the study?
Do the questions include words like compare or correlate that indicate a
quantitative study?
Do the questions include identification of the independent and dependent
variables?
Does the hypothesis fit with the research questions?
Do the questions and hypothesis identify the participants for the research study?
Do the questions and hypothesis specify the participants and the research site?
Relevant Scoring Guide Criteria
•
•
Formulate a quantitative research question and associated hypothesis.
Communicate in a manner that is scholarly, professional, and respectful of the
diversity, dignity, and integrity of others and is consistent with expectations for
health care professionals.
Part 2: Quantitative Methods and Data Collection
For this second part of the assignment, you will draw on your readings about qualitative
methods and data collection to first identify specific quantitative methodologies that will
enable you to answer your research questions. Second, you will discuss specific qualitative
data collection tools or strategies that are appropriate for this purpose. Questions to
consider when looking for strategies and tools include the following:
•
•
•
What is your research question specifically trying to answer?
How does the specific HAC affect the types of tools and strategies that would
work best?
How does the health care setting where the research project will occur affect the
types of tools and strategies that would work best?
This part should also include a review of why it is important to a potential research plan as
a whole to ensure that your data collection plan is relevant to your chosen methodology.
This section should be 2–5 pages long, depending on the level of detail required to fully
present your methodologies and data collection tools and strategies.
Relevant Scoring Guide Criteria
•
•
•
•
Identify relevant quantitative methodologies that will support answering a
research question.
Describe quantitative data collection tools or strategies that are appropriate for
answering a research question.
Explain the importance of methodologically relevant data collection to a
research plan as a whole.
Communicate in a manner that is scholarly, professional, and respectful of the
diversity, dignity, and integrity of others and is consistent with expectations
for health care professionals.
Additional Submission Requirements
